Job Description:
We are seeking a compassionate and driven Social Worker to join
our Skilled Nursing team in CO . In this role, you will support
residents and families through the continuum of care by providing
psychosocial assessments, care planning, discharge coordination,
and advocacy services. Working collaboratively with the
interdisciplinary team, you will help ensure each resident’s
emotional, social, and transitional needs are met in accordance
with CMS and Colorado state regulations. This position plays a
critical role in enhancing resident quality of life, supporting
family communication, and ensuring smooth transitions of care.

comprehensive psychosocial assessments for residents upon admission
and as needed throughout their stay Develop, implement, and update
individualized care plans in collaboration with the
interdisciplinary team Participate in care plan meetings and
contribute to the MDS process, including psychosocial documentation
Coordinate discharge planning, including referrals to community
resources and post-acute services Provide emotional support and
counseling to residents and families regarding adjustment, illness
progression, and end-of-life concerns Advocate for resident rights
and assist in resolving concerns or grievances Facilitate family
communication and support informed decision-making Maintain
accurate, timely documentation in accordance with CMS, OBRA, and
Colorado state regulations Assist with advance directives and care
transition planning Collaborate with nursing, therapy, and
administration to promote quality outcomes and resident
satisfaction Qualifications Bachelor’s Degree in Social Work (BSW)
required; Master’s Degree (MSW) preferred Active Colorado Social
Work license (LSW or higher) required, or ability to obtain prior
to hire Minimum one (1) year of social work experience in a
healthcare setting preferred Skilled Nursing Facility (SNF)
experience strongly preferred Knowledge of CMS regulations, OBRA
requirements, and Colorado long-term care standards Experience with
discharge planning and interdisciplinary care coordination Strong
documentation and assessment skills Excellent communication,
conflict resolution, and advocacy abilities Ability to maintain
