Compensation & Scheduling: Part-Time: Up to 24 visits per week
Competitive Pay: $70.00 – $85.00 per routine visit Flexible

meeting your professional goals Key Responsibilities: Develop and
implement individualized treatment plans Conduct assessments using
standardized tools and clinical judgment Provide therapy focused on
mobility, strength, balance, and safety Educate patients and
caregivers on home exercise programs and adaptive strategies
Maintain accurate and timely clinical documentation Collaborate
with a multidisciplinary care team to ensure coordinated patient

organization. Requirements Licensed Physical Therapist in Colorado
Experience in home health or a related field Strong communication
and interpersonal skills Proficient in electronic medical record
documentation Ability to develop and implement individualized
