#36763Job Summary:The Patient Services Representative will be a
part of a comprehensive psychosocial team that supports pregnant
adolescents during their prenatal care at UCHealth Anschutz. The
patient age range is from 12-20, and most patients have experienced
stressors related to social determinants of health. The Patient
Services Representative's responsibilities will include collecting
specific information from patients and families, working
collaboratively with a multi-disciplinary team to advance the
designated care plan and recommendations, facilitating connection
between patients and families with professionals internal and
external to the institution, communicating with community agencies
to facilitate referrals, and maintaining an organized resource
repository. Additional responsibilities include working with
patients, families, providers, and care teams to identify and
address barriers to accessing services, resources, and supports
that enhance the health and well-being of patients and families.
The Patient Services Representative will enter data obtained from
each distinct point of contact into the secure database management
system and/or medical record. A goal of this position is to improve
the coordination, integration, and access to healthcare and related
services for patients and families to ultimately enhance their
health and well-being. Key Responsibilities:
- Responsible for coordinating the care plans, making certain all
members of the team are communicated with, and that the patients
can be scheduled in a timely manner into our referred
agencies/programs/community-based services.
- Responsible for patient-facing activities, including program
enrollment, answering phone calls, responding to patient messages,
connecting to resources, and patient education as directed by the
multidisciplinary team.
- Performs daily review of clinic schedules and miscellaneous
clerical duties to ensure proper patient flow in the clinic.
- Create and maintain referral tracking reports to ensure
patients are connecting to the necessary services.
- Event management for monthly baby shower, including own
transportation for the pickup and drop off of baby supplies. Work
Location:Onsite - this role is expected to work primarily onsite,
with the potential of working 1-2 days remotely, and is located in

Qualifications:
- At least one (1) year of patient/client-facing experience.
- Substitution: A combination of education and related
technical/paraprofessional experience may be substituted on a
year-for-year basis.Applicants must meet minimum qualifications at

- Bachelor's Degree
- Experience in a healthcare setting or a community-based
organization.
- Experience managing and handling sensitive and confidential
information.
- Experience working with students, staff, and faculty within a
higher education and/or academic medical environment.
- Experience working with EPIC or another electronic medical
system.
- Experience with case management, care coordination, or systems
navigation.
- Experience working with individuals with pregnant or parenting
adolescents.
- Bilingual or Multilingual - able to perform job
responsibilities in English and another language(s).Knowledge,

- Knowledge of community health care systems and/or organizations
that support physical or behavioral health.
- Strong interpersonal and patient care skills with the ability
to establish rapport with and provide high-quality care to persons
of diverse backgrounds.

supervision.
- Collaborative approach to multi-disciplinary patient care and

Range:The starting salary range (or hiring range) for this position
has been established as $37,944 to $44,015.The above salary range
