Nurse Navigator - Oncology, Highlands Ranch University of Colorado Anschutz Medical Campus Department: Community Practice Medicine Job Title: Nurse Navigator - Oncology, Highlands Ranch Position #: - Requisition #: 29760 Job Summary: The Nurse Navigator (NN) will be a member of the care team that will be focused on providing personalized one-on-one patient care. The NN assesses and evaluates patient conditions, and then plans, implements, provides, and documents patient care in a manner that follows professional nursing standards and outlined practices. The NN will assist in expediting the process for evaluation of clinic patients through a multidisciplinary team approach, coordination of services and scheduling exams, optimizing patient care outcomes. The NN will help the patient navigate clinic visits, follow-up testing, diagnostics, labs and other services, coordinates care to reduce gaps in understanding, counsels and educations patients and families, and ensures high quality patient outcomes. Key Responsibilities: Facilitates patient intake by phone, email or online through Epic MHC. Triages, coordinates care, and orders appropriate patient testing under University of Colorado School of Medicine protocols and guidance. Acts as Liaison between patient and clinical staff, administration, physicians, managed care companies, community/external resources. Facilitates resolution to patient care gaps and follows up by both proactively reaching out to patients/families, and continuously addressing direct phone calls and messages from patients. Participates in relevant quality workgroups to assist in the development of patient education materials, clinical pathways, and workflows to improve patient and provider compliance with meeting health measures and meeting the needs of the patients/families. Substitution: Equivalent professional nursing experience will be considered in lieu of degree. Registered Nurse (RN) license in the State of Colorado. At least three years of nursing experience. Applicants must meet minimum qualifications at the time of hire. Conditions of Employment: Current Basic Life Support (BLS) for Healthcare Providers issued by American Heart Association or American Red Cross healthcare provider level CPR certification. Must be able to work in-person. Must be able to travel to alternate locations as assigned. Preferred Qualifications: 2 years job-related/clinic experience. RN experience in Specialty Area. Bilingual, Spanish-speaking. Nursing certification (ANCC) in specialty area. Knowledge, Skills, and Abilities: Ability to effectively communicate at all levels, including physicians, leadership, and staff within the various affiliate organizations (UCH, Children's Colorado, CU Medicine, CU SOM). Ability to collaborate with multidisciplinary teams and respond to time-sensitive situations. Ability to work within large, complex healthcare systems. Ability to analyze and interpret data. Ability to compose letters and memorandums. Ability to deal calmly and courteously with people.
