Job Description:
POSITION SUMMARYThe Nurse Practitioner provides treatment for
patients with substance use disorder along the lifespan in an
outpatient setting. Treatment includes assessing, planning,
implementing, coordinating, and evaluating the patient care among a
multi-disciplinary team.PRIMARY JOB RESPONSIBILITES- Consulting
with patients to understand their symptoms and health concerns.-
Working in a collaborative manner with the Medical Director.-
Discriminating between normal and abnormal findings on the history,
diagnostic tests; initiates referral and consultation when
appropriate.- Regulating, administering, and adjusting medications
and evaluates client's response to medication regimen and adjusts
as appropriate.- Providing consultation and education to members,
families, staff and community on medication actions, side effects,
and proper health and nutrition habits.- Prescribing or
administering medication, therapy, and other specialized medical
care to treat or prevent illness, disease, or injury.- Explaining
procedures and discussing test results or prescribed treatments
with patients.- Monitoring patients' conditions and progress, and
re-evaluating treatments, as necessary.- Providing health and
wellness advice to patients, including diet, physical activity,
hygiene, and disease prevention.- Maintaining detailed notes of
appointments with patients, including comments, tests, and
treatments prescribed, and test in the required EMR- Adheres to all
regulations and CMS and P&P and best practices- Reviews PDMP
and documents findings in medical case notes at every visit-

Referring patients to another medical specialist, whenever
necessary.- Preparing official health documents or records, when
necessary.- Completing all clinical documentation on the same day
as the encounter. Documentation must be complete, appropriate, and
using the required visit templates, and entered into the Electronic
Health Record (EHR).- Making referrals to appropriate primary
health care physician for management of acute and long-term health
problems.- Practicing within the standard of care, as determined by
the Chief Medical Officer, at all times.- Is familiar with and
adheres to CMS Policies and Procedures.- Reviewing the Prescription
Drug Registry of each patient at the time of their visit and at
other occasions when clinically appropriate.- Appropriate
professional appearance and behavior towards patients and staff.-
Adhering to professional ethics and boundaries in dealing with
patients and staff.- Maintaining a schedule of patient appointments
with efficient use of time.- Being present in the clinic on time
during all assigned hours.QUALIFICATIONS:Education, Certification
and Experience Requirements- Bachelor's degree in nursing and a
nurse practitioner master's degree with focus on psychiatric
nursing preferred not required- Three to five years' clinical
experience preferred- Must have fluency in documenting into an
electronic medical record- Must have active CPR and First Aid
certifications- Excellent communication skills- Possess a valid
fingerprint clearance card and have a recent tuberculosis test on
file- Superior analytical and problem-solving abilities

phone, computer, printer, and copier is required.- Frequent use of
Microsoft office products, including but not limited to Outlook,
Word, Excel, and PowerPoint.- The ability to use the internet and
various web browser software is required.Physical Working
Conditions and Office Setting Description- Requires sitting and
standing associated with a normal office environment.- Manual
dexterity using a calculator and computer keyboard.- Requires
prolonged sitting, standing, frequent bending, stooping, or
stretching.- Some lifting may be required.- Frequent and prolonged
typing and frequent and prolonged operation of computer, keyboard,
and telephones required.- Requires occasional use of fax machines,
telephones, copiers, and other office equipment.- Employee must
regularly lift and/or move up to 10 pounds, frequently lift and/or
move up to 25 pounds and occasionally lift and or move up to 50
