For more information, visit ( .Nature of WorkThe purpose of the position is to facilitate afterlife matters for patients who die while in the care of University of Colorado Hospital. Assistant should have the ability to facilitate all aspects of the death including assisting hospital staff through legal reporting requirements, aiding families after a death occurs, coordinating with physicians on death certificate completion, tissue donation, autopsy coordination, and ensuring that bodies are released to the correct entities.Professional FieldHealth CareSupervision ReceivedThis position reports to the Decedent Affairs SupervisorSupervision ExercisedNoneExamples of Work PerformedEssential Duties: List major job functions from most important to least important.Death Calls:
      • Respond in person to the nursing unit to aid hospital staff and meet with families.
      • Ensure all adult/infant deaths are reported to Adams County Coroner
      • Ensure all adult and infant deaths are reported to the Donor Information Line to determine the patient's eligibility for organ and/or tissue donation
      • Ensure decedents are released to appropriate family member, funeral home, coroner, or donation agency.
      • Maintain current knowledge of state statutesFamily Conference
        • Identify the legal next of kin
        • Discuss possible coroner intervention, autopsy, donation and funeral arrangements with the legal next of kin
        • When requested/pre-arranged, coordinate evaluation for total body donation to the State Anatomic Board
        • Offer the family guidance to social services, funeral financial assistance and grief counselors/groups as needed
        • Body Disposition
        • Coordinate with physicians to get death certificate (electronic process) prepared and signed within 48 hours of death.
        • Coordinate autopsy with the Pathology Department
        • Coordinate organ/tissue recovery with the appropriate transplant or donation banks
        • Coordinate with funeral home, coroner, State Anatomical Board or Donor alliance for removal of body
        • Deliver signed consents and copy of death certificate (if prepared) to Medical Records.Other Duties: List duties that may occur occasionally.
          • Create computer file for each deceased patient
          • Send a sympathy card
          • Coordinate viewings for patients who die without loved one's present
          • Prepare and distribute Discharge by Death procedure packets, maintaining proper levels on each unit.
          • Conduct training with staff with regards to the role of Decedent Affairs Office as needed
          • Attend seminars and in-services
          • Help arrange autopsies and tissue recoveries on University of Colorado Hospital patients who die outside of UCH
          • Coordinate with UCH physicians on completing death certificates for patients who die outside of the hospital
          • Field phone calls
          • Prepare and submit death certificates, intentional termination of pregnancy reports and state cremation on all fetal deaths
          • Collate statistics for monthly reports on Expired Patients and the Donor Lists sent to designated/requesting departments and agencies.
          • Assemble quarterly and annual statistical reports
          • Knowledge of medical terminology and medical abbreviations.
