Job Description:
Summary/Objective The Patient Care Coordinator I is the point of
contact to ensure patients' needs are met. The PCC I coordinates
patients' treatment plans and follow up on their needs. Essential
Functions
- Determines insurance eligibility and coverage for a variety of
plans and patient-specific situations
- Submits authorization requests to insurance in an accurate and
timely manner
- Communicates with patients and families to assess supply
needs
- Screens/rescreens patients for nutrition risk per established
criteria
- Documents data collection results and makes appropriate
referrals to the Dietitian as per
- Communicates effectively with physicians and dietitians
regarding patient needs and required documentation
- Works directly with staff, patients, insurance companies and
referral sources to ensure the best possible outcomes for our
patients
- Documents communication and action plans thoroughly
- Troubleshoots product sourcing issues if applicable
- Special Projects as assigned
- Process customer orders, requests and information updates into
a patient order management system
- Verify insurance coverage for all insurance carriers.
- Work with vendors staff to confirm order accuracy and details,
and occasionally ship orders as neededRequired skills
- Experience recognizing the needs and concerns of
individuals
- Effectively communicate orally and in writing
- Ability to follow verbal and written instruction

- Ability to demonstrate and educate to patient family members on
