Job Description:
Summary:
Conducts and documents socio-economic review and screening to
support the coordination of care. Under the supervision of a
registered nurse, provides indirect patient care in accordance with
applicable scope and standards of practice and with the practice
and with the policies, values, and mission of the organization.
Work Schedule: Full Time Days
Responsibilities:
Coordinates with home care agencies, post-acute care facilities,
durable medical equipment companies, transportation agencies and
others as indicated. Assists clients and patients in connecting
with appropriate Physicians, insurers, Payer programs, and other
resources to meet social and clinical needs.
Collaborates with the care management team for effective patient or
client outreach, care coordination and hand-overs across the
continuum of care. Completes referrals, provides educational
resources and transitional care arrangement in support of Care
Managers and Social Workers for patients recently discharged from
UCHealth facilities.
Retrieves record, imaging and diagnostic results, pathology and
other requests for treatment teams. Assists in generating reports.
Documents care provided per established guidelines. Maintains
patient care records according to hospital and HIPAA standards.
Contributes to the assessment of patients by collecting data for
analysis by an RN. Monitors, documents, and communicates patient's
status and reactions to interventions. Arranges care conferences as
needed. Supports multidisciplinary team meetings as assigned. Works
with internal and external resources to coordinate placement,
discharge planning, case management, and follow-up care.
Requirements:
The pay range for this position is: $22.39 - $34.70/ hour. Pay is
dependent on applicant's relevant experience.
An annual bonus may be paid to eligible employees based upon
