**Nature of Work**
Jobs in this career family are responsible for performing a wide
range of professional duties to support the university's teaching,
research and/or service missions through the provision of health
and wellness services, dealing with general, indirect patient care
operations and management of healthcare services. Functions include

**Position Emphasis:** The Division of General Internal Medicine
at the University of Colorado School of Medicine is seeking a
Healthcare Professional. This position will be in the CU Medicine
Internal Medicine-Cherry Creek clinic, a large ambulatory Internal
Medicine practice that cares for patients age 18 and older with
complex medical conditions. The Healthcare Professional will
support the goals of the Internal Medicine Clinic by direct and
indirect patient care under the direction of the Medical Director.
The Healthcare Professional will provide support for the clinical
mission through the Mental Health Care Coordinator role, as well as
other outreach and care coordination programs.
**Examples of Work Performed:** Work duties include conducting
patient outreach calls, running and interpreting reports,
communicating with primary care providers and other members of the
healthcare team, assisting patients in following-through with their
treatment plan, and documenting in the electronic health
**Mental Health Care Coordinator**
+ Conduct outreach calls to patients referred by providers who
need assistance accessing mental health resources in the community,
who have started a new medication, who have had a change in the
dose of their medication, or who have had a referral placed to
therapy or psychiatry.
+ Assist patients in locating mental health resources in
accordance with their insurance benefits.
+ Administer baseline and follow-up questionnaires for patients
who are engaged with clinic therapist, have recently started a new
medication, or had a change to their existing medication. This may
be accomplished via a phone call or electronically through the
+ Confirm patients have followed through on referrals to therapy
or psychiatry. Assist with barriers if patient has not been able to
secure an appointment.
+ Ensure timely follow-up appointment with primary care
+ Schedule initial and follow-up appointments for clinic mental
+ Communicate status updates with primary care provider between
visits with patient.
+ Communicate changes to treatment plan with patient between
visits with their primary care provider.
+ Pend orders in Epic as directed by the primary care
+ Ensure mental health screenings are conducted at appropriate
intervals and documented accurately in the electronic health
+ Run reports and identify patients who may need outreach based
+ Document patient encounters in the electronic health
+ Coordinate mental health care with other care team members as
+ Support clinic leadership in establishing protocols for mental
health coordinator program.

**Salary and Benefits:**
The salary range (or hiring range) for this position has been
established at $51,175 - $65,094.
The salary of the finalist(s) selected for this role will be set
based on a variety of factors, including but not limited to,
internal equity, experience, education, specialty and training.
The above salary range (or hiring range) represents the
University's good faith and reasonable estimate of the range of
possible compensation at the time of posting.
