**Nature of Work**

Jobs in this career family are responsible for performing a wide range of professional duties to support the university's teaching, research and/or service missions through the provision of health and wellness services, dealing with general, indirect patient care operations and management of healthcare services. Functions include hospital administration, healthcare regulatory compliance, patient relations, operational services, admissions, and medical records. Includes professional assignments in medicine, dentistry, nursing, pharmacy, counseling and behavioral health, public health, dietetics and nutrition, other allied health fields, alternative medicine and therapies, and related disciplines.

Professionals at the intermediate level are responsible for exercising discretion, analytical skill, personal accountability and responsibility in a wide range of areas including academic, administrative, managerial and student services functions. Work involves creating, integrating, applying and sharing knowledge directly related to a professional field.

At the intermediate level, duties may be more limited in scope and are performed with guidance and direction from other professionals.

**Position Emphasis:** The Division of General Internal Medicine at the University of Colorado School of Medicine is seeking a Healthcare Professional. This position will be in the CU Medicine Internal Medicine-Cherry Creek clinic, a large ambulatory Internal Medicine practice that cares for patients age 18 and older with complex medical conditions. The Healthcare Professional will support the goals of the Internal Medicine Clinic by direct and indirect patient care under the direction of the Medical Director. The Healthcare Professional will provide support for the clinical mission through the Mental Health Care Coordinator role, as well as other outreach and care coordination programs.

**Examples of Work Performed:** Work duties include conducting patient outreach calls, running and interpreting reports, communicating with primary care providers and other members of the healthcare team, assisting patients in following-through with their treatment plan, and documenting in the electronic health record.

**Mental Health Care Coordinator**

+ Conduct outreach calls to patients referred by providers who need assistance accessing mental health resources in the community, who have started a new medication, who have had a change in the dose of their medication, or who have had a referral placed to therapy or psychiatry.

+ Assist patients in locating mental health resources in accordance with their insurance benefits.

+ Administer baseline and follow-up questionnaires for patients who are engaged with clinic therapist, have recently started a new medication, or had a change to their existing medication. This may be accomplished via a phone call or electronically through the patient portal.

+ Confirm patients have followed through on referrals to therapy or psychiatry. Assist with barriers if patient has not been able to secure an appointment.

+ Ensure timely follow-up appointment with primary care provider.

+ Schedule initial and follow-up appointments for clinic mental health therapist.

+ Communicate status updates with primary care provider between visits with patient.

+ Communicate changes to treatment plan with patient between visits with their primary care provider.

+ Pend orders in Epic as directed by the primary care provider.

+ Ensure mental health screenings are conducted at appropriate intervals and documented accurately in the electronic health record.

+ Run reports and identify patients who may need outreach based on criteria.

+ Document patient encounters in the electronic health record.

+ Coordinate mental health care with other care team members as indicated.

+ Support clinic leadership in establishing protocols for mental health coordinator program.

**Other outreach and care coordination**

+ Conduct patient outreach calls to support clinical care initiatives.

+ Communicate with providers about clinical care initiatives.

+ Schedule appointments with primary care provider as indicated.

+ Run and interpret reports.

+ Provide supportive documentation in the electronic health record as indicated.

+ Maintain a working knowledge of healthcare quality measures and care requirements needed to satisfy the measures.

+ Support primary care providers in achieving care requirements for their patients.

+ Other duties as assigned.

**Supervision Received:** Responsible to the GIM Social Work Supervisor. Must be able to work collaboratively and proactively with the members of the clinic leadership team, faculty, and staff.

**Salary and Benefits:**

The salary range (or hiring range) for this position has been established at $51,175 - $65,094.

The salary of the finalist(s) selected for this role will be set based on a variety of factors, including but not limited to, internal equity, experience, education, specialty and training.

The above salary range (or hiring range) represents the University's good faith and reasonable estimate of the range of possible compensation at the time of posting.

**Qualifications**

**Minimum Requirements:**

+ Bachelor's degree social sciences, business fields, science, humanities, communications or related fields

+ One year of ambulatory healthcare experience

**Required: Applicants must meet minimum qualifications at the time of hire.**

**Preferred Requirements:**

+ Experience working as part of an interdisciplinary healthcare team

+ Experience with electronic medical records, Epic preferred

**Knowledge, Skills, and Abilities:**

+ Computer Skills Needed

+ Excellent computer literacy including experience with Microsoft Office Suite (including Excel, Word, PowerPoint, and Outlook)

+ Basic knowledge of electronic medical records systems (i.e. Epic) preferred

**General Skills**

+ Must have excellent communication skills and enjoy interacting with patients, health care providers, and care teams

+ Must be able to work with minimal supervision

+ Must be creative and proactive

+ Must be professional, responsible, and persistent

+ Must be able to think critically and find solutions to a variety of problems

+ Customer service experience a plus

+ Must be comfortable working with patients experiencing depression, anxiety, or other mental health conditions

+ Interest in mental health
