Job Description Job Description Family Nurse Practitioner (FNP) Location: Longmont, CO (On-Site | Occasional Boulder Support) Schedule: Monday–Thursday 8AM–5PM | Friday 8AM–2PM Salary: $125,000–$150,000 annually, based on experience, clinical expertise, and background in preventive or functional medicine About Cloud Medical We are a fast-growing Direct Primary Care (DPC) practice delivering proactive, preventive, and longevity-focused primary care outside the traditional insurance system. Cloud physicians and NPs practice with: Longer visits Lower appointment volume (13–15 scheduled per day) Real clinical autonomy Direct patient relationships No insurance pressures If you’re excited about proactive medicine, prevention, and meaningful care — this is your opportunity. What You’ll Do See 13–15 scheduled patients per clinical day Provide comprehensive primary, urgent, and preventive care Build long-term patient relationships Perform gynecology exams, injections, and routine in-office procedures (including HRT) Support preventive screenings and clinical interventions through diagnostics recommendation & review Deliver personalized, optimization-focused treatment strategies Complete encounter notes within 24–48 hours Partner with Medical Assistants to reply to patient messages in a timely, professional manner Partner with Medical Assistants during triage and procedures Participate in case reviews, team huddles, and clinical meetings Provide scheduled on-call support per provider rotation You Will Thrive Here If You: Love patient interaction and relationship-based care Value autonomy and thoughtful decision-making Feel energized by and knowledgeable of preventive and longevity medicine Enjoying sharing ideas and want to improve care delivery This Role May Not Be a Fit If You: Prefer high-volume, insurance-driven environments Want strictly protocol-based, task-focused work Prefer minimal patient engagement Are not interested in helping shape a better model of care Qualifications (Required) Active Family Nurse Practitioner (FNP) license Board Certified FNP Primary care experience Strong independent clinical judgment Excellent communication skills Working on-site in Longmont and Boulder clinics Preferred Experience Direct Primary Care or concierge medicine Women's health experience Functional or longevity medicine Compensation & Benefits $125,000 – $150,000 annual salary Paid Time Off Paid Holidays Paid Sick Leave 401(k) Professional and personal growth support Company Description About Cloud Medical We are a fast-growing Direct Primary Care (DPC) practice delivering proactive, preventive, and longevity-focused primary care outside the traditional insurance system.
