Job Description:

*Join Our Compassionate Hospice Team *Advanced Home Health and Hospice of Colorado is looking for a dedicated and compassionate *MSW* with a heart for supporting patients and families. In this role, you will provide *emotional support, resource coordination, and advocacy* for hospice and home health patients and their families.*Responsibilities:** Conduct psychosocial assessments and develop individualized care plans.* Provide emotional and grief support to patients and their families.* Assist with advance care planning and resource procurement.* Collaborate with the interdisciplinary hospice team or home health team to ensure comprehensive care.* Assist with volunteer management, including recruiting, training and coordinating hospice volunteers.* Maintain accurate and timely documentation.*Qualifications:** Masters degree in Social Work (MSW) required* Experience in hospice, home health, palliative care, or medical social work preferred* Strong interpersonal, communication, and advocacy skills* Compassionate and patient-centered approach* Excellent written and verbal communication skillsJob Type: Part-timePay: $60,000.00 - $70,000.00 per yearBenefits:* 401(k)* Dental insurance* Health insurance* Life insurance* Mileage reimbursement* Paid time off* Vision insuranceMedical Specialty:* Home Health* Hospice & Palliative MedicineSchedule:* Monday to Friday* Weekends as neededEducation:* Master's (Required)Experience:* Hospice or Home Health: 1 year (Preferred)Work Location: On the roadby Jobble
