Job description We are currently seeking a Full-time/Part-time
Home Health Physical Therapist to join our team. Overview : We are
looking for coverage in the South Aurora and surrounding areas.
Amazing Care Company, is an accredited pediatric therapy home
health agency that has been passionate about serving the greater
Denver area for over 15 years. We are a family centered company
consisting of a strong and close team of speech pathologists,
occupational, and physical therapists, working together in
partnering with families, physicians, schools, and other
professionals in providing the highest quality therapy to the
pediatric population within the home setting. We have full time
office staff to handle the administrative piece of providing
therapy, allowing our therapists to focus on clients and their
families. Our therapists specialize in providing therapy for
children of all ages presenting with a variety of abilities and
needs. We are currently looking to hire a part time Pediatric
Certified Physical Therapist for both the southern and also the
northern parts of the greater Denver Metro Area. There are also
opportunities for a mixed pediatric/adult caseload. GENERAL
STATEMENT OF DUTIES: Plans and implements specific treatment
programs for individual clients according to the principles and
practices of Physical Therapy (PT) and according to the orders
coordinated with a licensed physician. DUTIES AND RESPONSIBILITIES:
Responsible for assessing, planning, implementing, evaluating and
coordinating care for clients in their home under the scope of
practice of PT.
Develops and implements appropriate treatment programs to address
problems identified; develops realistic personal goals with client;
re-evaluates, sets new goals and adjusts treatment program
Assesses need for adaptive equipment. Instructs clients, family
members and other home health team members in the care and use of
the equipment. Develops needed plans for modifying
equipment/appliances and physical surroundings in the home to
Work in conjunction with area orthotists and/or equipment
representatives for adaptive bracing and equipment.
Observes, records and reports to physician and/or supervisor,
client's reaction to treatment, client's progress and any changes
in client's condition.
Coordinates other service team members, both in formal and informal
care conferences, to assure sufficient flow of information to
support care of client. EDUCATION AND EXPERIENCE: Graduate of an
accredited Physical Therapy program. MPT or DPT preferred.
Current BLS certification.
Has current State of Colorado Physical Therapy license.
Minimum 1year of experience or fieldwork completed in pediatric
therapy required. Job Types: Full-time, Part-time Pay: Up to $80
per Visit and Sign on bonus: $2000 Full-time/$1000 Part-time
