hour shifts. Rotating Saturdays 6 hour shifts; On call
requirements/shifts approx. One shift a week 6p-7a & rotating
weekends. Weekends are Fri-Monday AM call.Job OverviewThe Dialysis
Nurse supports Children's Hospital Colorado (CHCO) mission, vision,
and core values. The Dialysis Nurse provides direct patient care
for patients suffering from acute or chronic kidney failure in
accordance with applicable scope and standards of practice with the
policies, values, and mission of the organization. Monitors and
manages patients during acute and chronic dialysis treatments to
maintain a safe environment, optimize outcomes, and maximize
available resources.ResponsibilitiesPOPULATION SPECIFIC CARE

Essential FunctionsAn employee in this position may be called upon
to do any or all of the following essential functions. These

be expected to perform. Responsible for the safe and effective
delivery of peritoneal and/or hemodialysis direct patient care
Develops and maintains strong professional relationships with
multidisciplinary healthcare providers to provide individualized
patient/family care Collects, monitors, and reports on quality of
care outcomes and measures; provides input into the continuous
evaluation and improvement of internal quality and safety
processes, procedures, and performance measures Collaborates with
patients, families, and providers to facilitate family meetings and
as needed to establish an effective plan of care based on mutually
developed goals and in an environment that fosters shared decision
making. Assess patient's and family's unmet health and social needs
at home or in other community locations and collaborates with
social work and family navigators as needed. Monitors patients'
adherence to plan of care and progress toward goals. Facilities and
documents change and/or interventions as needed Provides education
and coaching to patients and families to improve health literacy
and work towards self-management of their health. Facilitates
patient access to appropriate medical and specialty providers as
well as medication acquisition and management, procedures/tests,
appointments, and referrals Serve as a point of contact, advocate,
navigator, and informational resource for patient, family, care
team, and community resources. Participates in the development and
revision of policies and procedures, as well as with quality
improvement and research activities impacting care for designated
patient populations Collect, monitors, and reports on water and
instrumentation quality and safety according to dialysis protocols
Works with manager to mediate problems involving patient, families,
and hospital personnel Ensures compliance with clinical policies,
procedure, and best practice in order to minimize the risk of harm
to patients and providers. Serves on department and
organization-wide committees as directed Other InformationSCOPE &

Guidelines are generally numerous, well established, and directly
applicable to the work assignment. Work assignment and desired
results are explained by general oral or written instructions.
Complexity: Falls between the entry and full performance/senior
level. Duties assigned are generally repetitive and restricted in
scope but may be of substantial intricacy. Employee primarily
applies standardized practices. Decision Making: Decisions or
recommendations on non-standardized situations are limited to
relating organizational policies to specific cases. Brings non
routine issues to supervisor. Problems that are not covered by
guidelines or that are without precedent are taken up with the
supervisor. Communications: Contacts with team members, clients or
the public where explanatory or interpretive information is
exchanged, gathered, or presented and some degree of discretion and
judgment are required within the parameters of the job function.
Supervision Received: Under normal supervision, within a
standardized work situation, the employee performs duties common to
the line of work without close supervision or detailed instruction.
Work product is subject to continual review. Qualifications
Education: Bachelor of Science in Nursing (BSN) Experience: One
year nursing experience Certification(s): BLS/CPR from the American
Heart Association with at least 6 months left before expiration is
required upon hire. Licensure(s): Registered Nurse (RN) license
Physical RequirementsAbility to Perform Essential Functions of the
Job Body Movement: Will frequently stand/walk up to 12 hours per
day. Will frequently use hands while keyboarding and writing. Will
frequently sit up to 2-4 hours per day while accessing computer,
talking on the telephone, and while attending meetings. Back and
Upper Body (lifting and carrying): Will occasionally lift/force
exerted up to 35 pounds/16 kilos. Vision: Constantly uses sight in
performance for daily job duties. Hearing: Constantly uses hearing
in performance for daily job duties. Others: Able to assist or
assume direct patient care as census directly permits. Exposure to
unsteady or falling patient: In the course of transferring a
patient, may encounter a patient who falls or loses balance,
causing employee to fall and/or bear the full weight of a patient.

hours worked per week): $69,191.84 to $103,787.76Hourly Salary
