Registered Nurses with a passion for Case Management! Our busy
Clinics are recruiting RN Case Managers! We need your caring and
compassionate nursing expertise to continue providing our patients
with top notch quality care. Key Responsibilities: Works
collaboratively with physicians, interdisciplinary teams, patients,
and families to promote positive patient outcomes. Performs a care
continuum process that assesses, plans, implements, coordinates,
monitors, and evaluates the options and services required to
support the individual's health needs utilizing skilled
communication, education, and resources to promote quality and
cost-effective outcomes. Provides focused support to various areas
such as utilization management, value-based performance team,
emergency department, acute, ambulatory and specialty care teams.

Qualifications: Bachelor's degree in nursing, health care, or a
related field. Substitution: A combination of education and related
technical/paraprofessional experience may be substituted for the
bachelor's degree on a year for year basis. 2 years Nursing
experience. Registered Nurse (RN), State of Colorado. Applicants
must meet minimum qualifications at the time of hire. Preferred
Qualifications: 3 years job-related/clinic experience. RN
experience in Primary Care or Care Management setting. Nursing
certification (ANCC) in specialty area. Bilingual,
Spanish-speaking. Computer skills: EPIC, Kronos. Experience with MS
Office: Outlook, Power Point, Word. Conditions of Employment:
Current Basic Life Support (BLS) for Healthcare Providers issued by
American Heart Association or American Red Cross healthcare
provider level CPR certification. Vaccinated against COVID-19.
Complies with applicable University of Colorado School of Medicine,
Community Practice mandatory education and training. Maintains all
required licensure and certifications for position. Must be able to
work in-person. Must be able to travel to alternate locations as
assigned. Knowledge, Skills, and Abilities: Ability to interpret,
communicate and document with proper medical terminology. Strong
knowledge of CPT & ICD-10 procedure coding. Strong knowledge of
value-based care and risk-based coding procedures in Primary Care.
Ability to adhere to all policies and procedures, including but not
limited to standards for safety, attendance, punctuality, and
personal appearance. Strong organizational and time-management
skills. Ability to maintain confidentiality and communicate in
person, by phone, or in writing. Ability to deal calmly and
courteously with people in stressful, fast-paced environments.
Ability to follow oral and written instructions and established
procedures. Ability to multitask efficiently and effectively.
Ability to accurately reconcile patient medications. Ability to
maintain quality control standards. Ability to identify problems
and recommend solutions. Ability to effectively communicate at all
levels, including physicians, leadership, and staff. Ability to
work within large, complex healthcare systems. Ability to attest to
computer proficiency and learn new computer applications. Ability
to keyboard/type, write legibly, and accurately record information.
Ability to analyze and interpret data. Ability to
recognize/interpret the physical exam process and record exam

starting salary range (or hiring range) for this position has been
established as: $69,511 to $88,419 The above hiring range
represents the University's good faith and reasonable estimate of
